Patients often present with a concern that they are frequently asked if they are sad or if they are angry. Almost universally, this is the result of a combination of frown lines between the brows and downturning of the corners of the mouth. With normal facial proportions, the line that forms where the upper and lower lips meet is virtually a perfectly horizontal line where the corners of the mouth are at the same level as the central portion.
In all patients to varying extents, the corners of the mouth tend to droop with time. Many people mistakenly believe that injecting fillers into this vicinity can somehow magically lift or elevate the corners of the mouth. The reality is that unless helium was injected, this is a fallacy. In 15-20 minutes under local anesthesia, 3-5 small sutures can be placed at the corners of the mouth that are removed within five days that result in reliable, consistent and natural-appearing straightening of the lip line.
This simple procedure has an extremely high satisfaction rate. The patient must accept the fact that they will have a pink scar for several weeks which will require makeup for coverage, but this tends to fade beautifully with time. This procedure, combined with fillers where necessary, may cause a very dramatic yet lasting and natural appearance of a more pleasing countenance. Although this is a procedure that is not widely performed because many surgeons and patients are fearful of conspicuous scarring, it is a procedure that we have been consistently performing for over 20 years on hundreds of patients with an outstanding satisfaction rate.
